Any group that subscribes to the purposes and policies of USLCA and has at least 25% members of USLCA among its Board of Directors may apply for affiliation.

USLCA Chapters will receive:

The 2011 annual dues for USLCA Chapters are structured as follows:

Membership Dues: January 1, 2011 – December 31, 2011

Less than 50 members: ……………….US$  100.00

50-99 members: ……………………… US$ 130.00

100 or more members: ………………..US$ 160.00

The application procedure includes proof of being a legally constituted organization, which has a bylaws, and articles of incorporation. Applications will include the following attachments:

• Chapter Name

• Chapter Agreement (signed and dated)

• Officers List

• By-laws and Articles of Incorporation.

• Mission Statement
• Goals and Strategic Plan

• Other optional information may be added (i.e. policy and procedure manual, newsletters, minutes, etc.)
